Immunizations If you’re under 22 years of age and plan to attend classes on-campus at UST, you’re required to have the Bacterial Meningitis vaccine.

It must have been received after your 16^th birthday and within the last 5 years prior to enrollment.

You won’t be able to enroll in or attend classes until these records are on file with the Office of Health Promotion and Wellness.

Is this required by law?

Yes. Texas Senate Bill 62 requires all entering students under the age of 22, including transfer and graduate students, to submit evidence that they have been vaccinated against Meningococcal Meningitis (bacterial meningitis). The vaccine required must be the MCV4 vaccine or equivalent, and NOT the Meningitis B vaccine.

NOTE: You will not be able to register for classes until the appropriate records are on file in the Office of Health Promotion & Wellness.

The meningitis vaccine must have been received within the last 5 years prior to enrollment. In accordance with the Center for Disease Control (CDC) recommendations, the vaccination must have been received at the least after your 16th birthday and within the past five years. This information DOES NOT apply if you're a student 22 years and older OR if you're enrolled only in online or other distance courses.

What if I’m unable to attain my records?

Then you must be vaccinated again. Your primary care physician can provide these, or we can recommend other locations to receive the necessary vaccinations.

Housing Immunization

Are my health records kept confidential?

Yes, all information is accessible only to the professional staff that processes and verifies these data for housing/enrollment purposes and will not be released without your written authorization.

All files are kept strictly confidential in accordance with HIPAA guidelines.

Whatever you submit must include the month, day and year the vaccination was administered.

We accept any of the following:

A UST immunization form*
An official immunization record from a state or local health authority*
An official record received from state officials, including a record from another state

*Must be signed or stamped by a certified, licensed physician or his/her designee, or public health personnel.

Immunization records can be submitted through your MYSTTHOM, using the Immunization Form Upload tile located on your homepage.

You may submit this in one of two ways:

A UST Bacterial Meningitis Immunization Medical Exemption Affidavit signed by a physician who is duly registered and licensed to practice medicine in the United States.*
A Texas Dept. of State Health Services Affidavit signed by the student stating that the vaccination is declined for reasons of conscience, including but not limited to a religious belief.

*Must state that, in the physician’s opinion, the vaccination required would be injurious to the health and well-being of the student
